Health Reach Announces New Chronic Disease Management and Education Program

Many of our new patients are presenting with serious chronic diseases including diabetes, hypertension and cardiovascular disease. In fact, HealthReach currently treats more than 700 persons with serious chronic disease. Looking ahead to 2009, HealthReach has risen over $80,000 to implement a new Chronic Disease Management and Education Program which was prototyped in the 4th quarter of 2008. This program is designed to significantly improve patient outcomes in the areas of diabetes, blood pressure and heart disease management. This new program will combine physician and specialists visits with new on-site laboratory services, pharmaceutical services and special patient education services to treat, monitor and follow up with patients to ensure the best possible care for our patients and possible disease control outcomes are achieved. Based on the success of this program, it is our hope to participate in a planned new county wide initiative with the Lake County Health Department and North Shore University Health Systems to increase access to chronic disease screening and management services to a much larger patient base.
